For public events, please have this form filled out at least three weeks before your event date.

If you'd like your event featured in our monthly studio newsletter, please have this filled out at least one week before the 1st of the next month.

Once this form is submitted, we will reach out within 24-48 hours with next steps.

Do you require any special setup requirements? (30 mins before and 30 mins after rental time period is included for you to setup/take down)

Insurance

Do you carry liability insurance for your practice?

Yes (certificate required 7 days before rental)

No, but I can obtain event insurance

No, I'd like to use the studio's insurance (each participant is required to fill out a waiver which the studio collects - this does not cover renters personal property).
